Pathways to Opportunities consists of 3 branches of support services – Care at Home, Community Support and Day Service. We are able to meet individuals’ needs in a flexible and person-centred way, and every support package is bespoke to the individual.

People can choose a service which suits them best, combining different types of support for variety and accessing opportunities. We do not have a specific criteria for referring to our service; if you are interested in receiving support from ourselves, you can contact us directly, through a social worker or family member and we can get the process started. For more information on how to access our services, please contact Aimee on 0161 652 6466, or email info@pathways-to-opportunities.co.uk

Day Service

Our Day Service is located in the heart of the community in Chadderton, just behind the Town Hall. We open from 9am-3.30pm Monday to Friday, and 12pm-5pm on Saturdays and Sundays
(These times can be flexible in accordance with your care package to best suit your needs – we can offer a bespoke service combining Day Service/1:1 support).

Arts and Crafts

We provide paints, pencils, paper and other materials to bring out the creative side in you. We particularly enjoy making crafts around special occasions such as Birthdays, Easter, Christmas and any other holiday we can celebrate!

Community Support

Our friendly, approachable and well-trained staff team can support individuals out in the community, whether that be to accompany them to appointments, explore opportunities and activities in the area, take part in exercise and hobbies, shopping, and more! Most of our staff team drive their own vehicles, but for those that do not they are happy to accompany people on public transport. Transport costs are not included in the price of our services and must be paid for separately.

Care at home

Pathways to Opportunities are registered with the Care Quality Commission to support people with their personal care as a regulated activity (please see latest inspection report and link to our CQC page below). We are also able to provide support in people’s homes with medication administration, and household tasks. We ensure to provide all of our Care at Home support in a person-centred way, and encourage those who we support to have their choices and dignity respected in all aspects of support, whilst encouraging independence as much as possible.
